Please download and submit the following five types of documents.
1.Summary of delivery
2. Collection of Forms (Form No. 1 to No. 5)
3. Facility use certificate (Form No. 4-1)
4. Accommodation certificate (Form No. 4-2)
5. Questionnaire
*Please submit the issuance application form (Form No. 1) at least 10 days before the scheduled tour date.
* After the tour, in the performance report (Form No. 4),
Please attach a certificate of use of tourist facilities (Form No. 4-1), a questionnaire, and other required documents.
